Our Mission
To help those with serious illness spend more quality days at home.
Our origin
Our founding vision was to make it easier for patients with serious illnesses to receive care consistent with their wishes. We built software for patients and their families to digitally create, save, and widely share goals of care documents for their providers to access. Now, hospitals and clinics nationwide use our software within their electronic medical records, enabling millions of patients to receive care they would like at the end of life.
Filling palliative care gaps
While supporting our advance care planning software, we consistently observed a lack of palliative care in the communities we served to meet the needs of patients and families. As a medical specialty, palliative care aims to reduce suffering and improve the quality of life for patients, caregivers, and families dealing with life-limiting, serious illnesses.
Bringing high-qualitycare home
We started a community-based palliative care practice for adults and children regardless of residence or income. We acquired ReSolutionCare, a cutting-edge, virtual-first palliative care practice in Northern California. With technology support, we developed a hybrid care model combining in-home and virtual visits to address patient needs and reduce avoidable ER or hospital visits. We have expanded our home-based care outside of California to four states in the western United States. Over the next twelve months, we plan to operate in five to seven additional states in the south and mid-west.
